This is to confirm that I would like to become a Muslim Funeral Services volunteer.

I understand that I have to abide by the rules and regulations of Muslim Funeral Services Ltd, as set out on the volunteer page of their website.

I understand that I have to take care and precautions to ensure my personal safety and avoid injury whilst performing any volunteering duties.

I understand that there are some risks in being a volunteer, which may include lifting, shifting, moving the deceased, preparation of the grave internally and being exposed to body fluids from the deceased during the washing and shrouding process. I understand that all these are undertaken in terms of Islamic tradition, and MFS will take all precaution to ensure my safety, but I do not hold MFS or its Directors responsible, for any injury sustained either physical or psychologically as a result of these actions.

I further understand that I MUST not perform any duties as soon as I become aware of any risk of possible injury at any time and advise a member of the organisation.

If my duties include driving, then I understand that I cannot drive if my licence is suspended for any reason, until it has been re-instated. I understand that I have a duty to disclose this information to the Directors of MFS.

In the event of an emergency, I consent to my next of kin being contacted using the above contact details.
