The purpose of this form is to record the condition of your apartment when you move-in, so that you can make note of any existing damages so that you are not billed for them when you move-out.
Please check all common areas (i.e., entry, living room, kitchen, laundry room, etc.), as well as your assigned bedroom and bathroom/vanity area carefully, and list wear-and-tear damages on this form. These issues can include, but are not limited to: stains in carpeting, scratches and dents in cabinetry or hard-case furniture (i.e., desks, tables, etc.), small holes in walls, etc. This form must be submitted within 48-hours of the first day of the semester classes (5:00pm on Wednesday, August 28, 2019) so it can be included in your apartment file.
IMPORTANT: Failure to complete this form could result in you being charged for damages at the end of the year.
